Sliding Fee Discount Program

Great healthcare shouldn't break the bank. CVH has three primary care clinics and one outpatient mental health clinic that offer convenient healthcare services at a discounted rate for patients who are uninsured or underinsured. These practices serve all patients regardless of their ability to pay. Discounts for essential services are offered based on family size and income. For more information, ask at the front desk. For services outside of these four locations, please refer to our Community Care Program described below.

Primary Care | Minden Village, Topaz Ranch Medical Clinic, CVH Senior Care and CVH Outpatient Behavioral Health Clinic serve all patients regardless of their ability to pay and offer a sliding fee scale.

What is a sliding fee scale?

Sliding fee scale is based on income. Click here to see the fees for services.

Services Covered:
Medical: The discount is applied to all in-office services at the above locations.
Pharmacy: Samples are provided without charge when available.
Lab & X-Ray: The discount is applied to in-office laboratory and x-ray services. Reference laboratory tests and consulting radiology interpretations are excluded.

A completed application including required documentation of home address, household income, and insurance coverage must be on file and approved by the business office at the clinic before the discount is granted. If the applicant is eligible for Medicaid, a written denial of coverage by Medicaid may also be required, however no patient will be denied services based on refusal to apply for other coverages.

Community Care

There are several financial assistance programs available in our area:

  • Medicare
  • Nevada Medicaid
  • CMSP or Medi-Cal
  • Douglas County Social Services
  • Lyon County Social Services
  • Carson City Human Services
  • SSI/SSD Social Security Income
  • Victim of Crime Program
If you do not qualify for any state-funded programs and are receiving services outside of the four clinics noted above (under Sliding Scale Discount Program), you may be eligible for CVH’s Community Care Program.

Our Patient Financial Advocate can assist in matching you with a program option that gives special consideration to your unique situation. Your income and assets are used to determine your ability to pay and are compared to the US Department of Health and Human Services guidelines. This service is free of charge for Carson Valley Health patients and all information will be kept confidential.
