Medical Records
The medical records office at Carson Valley Medical Center is located in the Administration Building behind the main hospital building, on the first floor. Office Hours are:
Monday - Friday 8:00 am to 4:30 pm
All requests for medical records require a medical release authorization form that can be obtained from the medical records office during business hours or can be downloaded from our website.

All requests for patient health information must be authorized by patient's signature or patient's representative. A copy of your drivers license or another valid form of identification is required.
Please allow five (5) business days for your request to be processed.
